Commercial insurance
Your health insurance is provided by your employer or you independently purchased it from a private health insurance company or the health insurance marketplace.
You may be eligible to save money with the ZEPOSIA 360 Co-Pay Assistance Program.
Some important details
- If you have commercial insurance and a prescription for ZEPOSIA, the Co-Pay Assistance Program may be able to offer financial support to help with the out-of-pocket costs for your treatment
- Eligible, commercially insured patients may pay as little as $0 a month for ZEPOSIA. By signing up, you are also agreeing to receive future communications about helpful tools and additional resources for your treatment

Government insurance
You are enrolled in a Medicare Part D plan, a Medicare Advantage prescription drug plan, or any other federal or state healthcare program.
If you have government-funded insurance, your coverage for ZEPSOSIA may vary.

Self-pay
You may choose to self-pay because you are uninsured, have insurance that does not cover your BMS medicine, or have higher out-of-pocket expenses for your BMS medicine through your insurance than under BMS Patient Connect. BMS Patient Connect gives you transparency into the cost of your medication.
- Eligible cash-paying U.S. patients with a ZEPOSIA prescription will be able to purchase the medicine via BMS Patient Connect with access to shipping across all 50 states and Puerto Rico
- Insurance is not accepted
- Review full program Terms and Conditions. Patients enrolled in a Medicare Part D plan, a Medicare Advantage prescription drug plan, or any other federal or state healthcare program (collectively, “government insurance”) must agree to the following conditions

In 1 business day, we’ll send a text confirming your prescription along with a checkout link to complete your order.

Step 2
Shipment
and delivery
1-2 business days after you check out, you’ll get shipment notification and tracking information, with delivery 3-5 days after checkout.
You can also find the tracking information by logging in to the portal.
Step 3
Refill
reminder
When it’s time, 21 days after checking out, you’ll receive an email reminding you to place a refill order.
