Your health insurance is provided by your employer, or you independently purchased it from a private health insurance company or the health insurance marketplace.

You may be eligible to save money with the SOTYKTU 360 SUPPORT Co-Pay Assistance Program.

Some important details

• If you have commercial insurance and a prescription for SOTYKTU, the Co-Pay Assistance Program may be able to offer financial support to help with the out-of-pocket costs for your treatment
• Eligible, commercially insured patients may pay as little as $0 a month for SOTYKTU. By signing up, you are also agreeing to receive future communications about helpful tools and additional resources for your treatment

Self-pay

You may choose to self-pay because you are uninsured, have insurance that does not cover your BMS medicine, or have higher out-of-pocket expenses for your BMS medicine through your insurance than under BMS Patient Connect.

BMS Patient Connect gives you transparency into the cost of your medication.

• Eligible cash-pay U.S. patients with a SOTYKTU prescription will be able to purchase the medicine via BMS Patient Connect with access to shipping across all 50 states and Puerto Rico
• Insurance is not accepted
• Review full program Terms and Conditions. Patients enrolled in a Medicare Part D plan, a Medicare Advantage prescription drug plan, or any other federal or state healthcare program (collectively, “government insurance”) must agree to the following conditions
