Your health insurance is provided by your employer, or you independently purchased it from a private health insurance company or the health insurance marketplace.

You may be eligible to save money with the ELIQUIS Co-Pay card.

Some important details

• If you have commercial insurance and a prescription for ELIQUIS, the Co-Pay card may be able to offer financial support to help with the out-of-pocket costs for your treatment
• Eligible, commercially insured patients may pay as little as $10 per 30-day supply or $10 for their first 90-day supply of ELIQUIS while enrolled in the program. Patients may pay as little as $30 for subsequent 90-day refills. Subject to maximum annual benefit of $2,000

Self-pay

You may choose to self-pay because you are uninsured, have insurance that does not cover your BMS medicine, or have higher out-of-pocket expenses for your BMS medicine through your commercial health insurance plan than purchasing directly from Bristol Myers Squibb.

ELIQUIS can be purchased directly from Bristol Myers Squibb through our ELIQUIS 360 Program utilizing CMM Patient Direct Pharmacy. Call ELIQUIS 360 Patient Support at 1-855-ELIQUIS (1-855-354-7847) to get started.

• Eligible U.S. patients with an ELIQUIS prescription may purchase their medication at a discounted rate directly through ELIQUIS 360 Support. This program offers uninsured and self-paying patients (patients who pay out of pocket for the full cost of their prescriptions) a straightforward way to access ELIQUIS, with clear and transparent pricing information
